To me it looks like the axle has shifted over and thats why you dont have enough threads to keep the nut tight. Loosen everything up and try to shift the axle back over to see if you can get some more threads to come out on the nut side? It may work.

The splines look fine as best as I can tell from the pic, so as long as the splines in the threaded collar are fine too, that isn't the problem. If you just put in new axle bearings and seals, make sure the seals have been pressed in correctly and aren't sticking out of the carrier.

Other than that, sometimes they are just a PIA to get back together. If you do have new seals and/or you've put a lot of grease on and inside the seals, it can try to squirt the axle back out the left side. I move the nuts down the collar until I can see the edge of the collar, then I push the collar in toward the rotor and put the little lock ring in the axle groove. If the axle keeps wanting to squirt out the other side and I can't get the collar to grab on the splines, I make sure the quad is on something solid, I sit down with the axle pointing toward me, put my right foot against the swingarm, grab the axle and pull it firmly until it stops, then I hold inward pressure on the collar and lock nut so they grab onto the splines while I turn the lock nut. Once the lock nut gets firmly against the lock ring, I release the axle and proceed to tighten the lock nut.
